What is the Micropeel?

As the average American becomes larger, it quickly becomes evident that it was far easier to put the weight on than to take it off. While it comes down to simple arithmetic — more calories burned than consumed = weight loss — it’s just not that simple for most people. Hence the growing demand for liposuction, where unwanted pockets of fat are reduced by basically vacuuming the fat out.

Dr. Hetzler performs tumescent liposuction for his patients in Little Silver and the surrounding areas because everyone, no matter what shape they’re in, wants to get rid of a little bit of fat.

What is liposuction?

Liposuction is a fairly well known cosmetic procedure, but there are some misperceptions. It came to the U.S. for the first time in the late 1980s after originating in France. The procedure that first debuted in the U.S. was quite violent, as the cannula (the tube with suction that is moved about under the skin to suction away the fat cells) was much larger than current models and it had to be moved back and forth aggressively to break loose the fat cells that were then suctioned out. These movements caused extensive tissue trauma and bleeding. This then resulted in extensive bruising that could linger for weeks before fully clearing.

Things have really changed today. Now, prior to inserting the cannula (which is very thin compared to the original models), Dr. Hetzler injects the area to be treated with tumescent solution. This consists of saline solution, lidocaine, and epinephrine. The saline solution helps with fluid loss. The lidocaine anesthetizes the area. And the epinephrine constricts both the blood vessels in the area for far less bleeding, and it constricts the fat cells, making them easier to break loose. That means Dr. Hetzler doesn’t need to be overly aggressive with the movement of the cannula. This allows him to finely contour the fat removed for a smooth, natural result. Plus, it results in far less tissue trauma. The bruising and recovery today are minimal when compared to those procedures done 30 years ago.

Where can I have liposuction done?

The question is really where can’t you have liposuction? Basically anywhere you have unwanted pockets of fat they can be removed with this procedure. Common targeted areas are the abdomen, flanks, buttocks, thighs, knees, upper arms, chin, and neck. Areas such as the chin benefit from today’s very thin cannulas.

Not for weight loss

It is important to remember that liposuction with Dr. Hetzler is not a weight-loss procedure. In fact, removing too much fat can be dangerous for a person. Liposuction is meant to remove stubborn pockets of fat that are almost impossible to get rid of through changes in diet or exercise. The lower abdomen is such an area. No matter how many crunches you do, once fat has accumulated there, it is very difficult to get rid of…without liposuction.

Dr. Hetzler wants his liposuction patients to usually be within 10 percent of their ideal weight to have this procedure at his practice.

Everyone gets a little stomach pooch at some point. If you’ve been pregnant and had a couple children, it’s almost a certainty. If you’ve gained and then lost a good deal of weight, same thing. General aging will also eventually deliver a small pooch as the muscles and skin in the area weaken. The issue is the skin — once it’s been stretched for a long period, it simply can’t tighten back down to its former degree of tautness.

That’s where Dr. Hetzler comes to the rescue with a tummy tuck. He’ll give you back your flat stomach once again.

What’s a tummy tuck?

The main job of a tummy tuck is to remove excess skin that has become loose and saggy. Some people think liposuction can remove a stomach pooch, but it can’t. Dr. Hetzler usually includes some amount of liposuction with his tummy tuck procedures, but liposuction cannot do anything about stretched, loose skin.

To remove the skin, Dr. Hetzler obviously needs to make an incision. Depending on how much of the abdomen area he seeks to address, the incision length can vary. For patients with more minimal sagging, a shorter incision along the bikini line will suffice. For those with universal sagging skin, the incision may circle the entire lower abdomen. This length of incision gives Dr. Hetzler access to the abdomen, flanks, lower back, and buttocks.

In a full tummy tuck, a second incision is usually necessary around the navel area to gain access to sagging skin above the navel. This incision is small.

What about fat?

Some tummy tucks include the removal of excess fat. Dr. Hetzler prefers to remove the excess fat in the center of the abdomen surgically and the excess fat of the sides with liposuction. He believes this delivers the smoothest, most accurate, and effective contouring of the area while still reducing the fat.

What about muscle?

Pregnancy often actually separates a woman’s stomach muscles, and they cannot come back to their former position without help. Dr. Hetzler brings those muscles back to the center in these procedures. This helps them support the back and usually also slims the waist.

Healing

Patients wear a custom girdle for 2-3 weeks after surgery. Bruising in the center of the abdomen will last 1-2 weeks. Areas that have received liposuction will stay bruised for up to 3 weeks. Pain in the abdomen from both the surgery and liposuction can last from 7-10 days. Most patients return to work in two weeks or so. Heavy lifting and strenuous exercise, however, must wait for 4-6 months.

One of the drawbacks to many breast augmentation procedures is the incision scar. This is the case when the patient opts to have her implants placed through the inframammary incision, located just above the breast crease on the underside of the breasts.

Many women when doing their initial research on the possibilities of breast augmentation don’t realize there is another option that doesn’t leave any scarring on the breasts. This option places the incisions in the armpits through the transaxillary incision.

What is transaxillary breast augmentation?

For these procedures, Dr. Hetzler makes the incision vertically in the armpit in the natural crease. He then inserts an endoscope, a narrow fiber-optic camera through the incision. This provides visuals of the anatomical structures and guides Dr. Hetzler throughout the procedure. This makes for much more precise implant positioning than with surgeons who do not use an endoscope in transaxillary augmentation. At this point, either the unfilled saline implant or the silicone implant is inserted through the incision and placed into the pocket created to hold the implant. At that point, saline implants are filled, and then are checked for size and placement. The fill tube is then removed and the implant automatically seals itself. The tube is withdrawn and the incision in the armpit closed. With silicone implants, Dr. Hetzler checks position and symmetry and then closes the incision. Then he moves to the other breast.

What are the advantages of this incision?

The main advantage of the transaxillary incision is that it does not create any scarring on the breasts. Because the incisions are made in both armpits and are made vertically in the natural crease, they become barely noticeable. This is quite a contrast to implants placed through the breast crease at the bottom of the breast, which creates a noticeable scar. This can be very important for patients with darker skin tones, as their scarring will likely be somewhat more visible going forward. Also, since Dr. Hetzler doesn’t have to cut the chest muscle or any breast tissue, recovery from this incision location is easier for most patients.

What are the disadvantages of this incision?

In most cases, only saline or smaller-sized silicone implants can be placed through the armpit incision. For some surgeons, the placement of the implants through the transaxillary incision can be more difficult due to the distance from the incision to the location of the breast pocket. Dr. Hetzler has extensive experience with this incision location, however, and has not had trouble with symmetry or other placement problems.

Carpal tunnel syndrome

The carpal tunnel is a narrow, fibrous passage in the wrist that protects the median nerve, which controls movement and sensation in the hand and thumb, index and middle fingers. If you’re feeling numbness, tingling, or weakness in your fingers, you could be suffering from carpal tunnel syndrome.

Carpal tunnel syndrome occurs when the median nerve is irritated by pressure. This can come from swelling or a change in tissue position that causes the carpal tunnel to squeeze and compress the nerve. The condition spreads gradually, usually beginning as an ache in the wrist. As it develops, tingling and numbness can move into the entire arm. Some people also begin to have weakness in the hand and arm when grasping small objects. One thing to note — it does not affect the pinky, just the thumb and three other fingers of each hand.

Dr. Hetzler performs carpal tunnel release surgery to create space for the median nerve.

Causes of carpal tunnel syndrome

Women are three times more likely than men to get the condition. That could be because women usually have a smaller carpal tunnel than men. Certain medical conditions can lead to it:

  • Obesity
  • Hypothyroidism
  • Rheumatoid Arthritis
  • Diabetes
  • Pregnancy
  • Trauma

Most people acquire carpal tunnel syndrome, however, through repetitive motion. Working certain jobs that involve repeating the same motion with your arm over a long time raise a person’s chances of getting it. These jobs include:

  • Assembly line worker
  • Sewer or knitter
  • Baker
  • Cashier
  • Hair stylist
  • Musician

Treatment

Generally, treatment involves lifestyle changes (change the repetitive motion, if possible) and stretching and strengthening exercises. It may help to immobilize the wrist with a splint. Anti-inflammatories or steroid injections can reduce the swelling. But if those have no effect surgery may be necessary because if left alone, the condition can cause permanent nerve damage.

Surgery

For Dr. Hetzler the goal is to cut the transverse carpal ligament and relieve pressure on the median nerve in the wrist. There are two approaches:

  • Open carpal tunnel release surgery. An incision is made at the base of the palm of the hand. This allows Dr. Hetzler to see the transverse carpal ligament that needs to be cut to release the pressure on the median nerve. 
  • Endoscopic carpal tunnel release surgery. A small endoscope is guided through a small incision in the wrist (single-portal technique) or at the wrist and palm (two-portal technique). The endoscope gives Dr. Hetzler a good view of the structures in the wrist without a larger incision.

Living in Little Silver, with lots of days at the beach over at Sandy Hook, we’re all exposed to a lot of sun. The sun’s ultraviolet rays are the stinkers, penetrating the skin and damaging its cells, leading to skin aging and wrinkling, and various skin cancers.

We all know that the best thing would be to never go outdoors, unless we are dressed like a mummy. But when you live so close to the water, who wants to do that?

So, we all apply sunscreen. But what do you really know about sunscreen and how it protects, or doesn’t protect, you? Here’s some info. If you have any questions about sunscreen and your skin, please talk with our resident skin expert, Christine Hetzler, RN, BSN.

Sun protection factor (SPF) and UV radiation

Since the advent of modern sunscreens, a sunscreen’s efficacy has been measured by its sun protection factor, now commonly known by its acronym, SPF. SPF is not really a measure of protection, it is a measure of how long it will take for ultraviolet B (UVB) rays to redden your skin compared to having no sunscreen on. For instance, SPF 15 means that it will take 15 times longer with sunscreen on than without. Why does it use the UVB rays? Those are the rays that cause sunburn because they penetrate only the epidermis, the surface layer of the skin.

As for amount of UVB protection, an SPF 15 sunscreen screens 93 percent of the sun’s UVB rays; SPF 30 protects against 97 percent; and SPF 50 protects against 98 percent. The U.S. Department of Health and Human Services recommends only SPF 15 and higher for providing adequate protection.

But scientists now say UVA rays, which penetrate far more deeply, into the dermis layer of the skin, also cause skin damage and skin cancer. So, you need a sunscreen that protects against both UVA and UVB. To do this, look for a sunscreen with at least SPF 15, plus some combination of the following UVA-screening ingredients: avobenzone, ecamsule, oxybenzone, titanium dioxide, and zinc oxide. When you see a label that says broad spectrum or multi spectrum or UVA/UVB these indicate that some UVA protection is provided. However, there isn’t a measure of how much protection these terms denote.

How do sunscreens work?

The ingredients in sunscreens form a thin, protective film on the surface of the skin and absorb the UVA radiation before it penetrates the skin. The “sunscreens” are actually physically screening the sun, meaning that their insoluble particles reflect the UV rays back off the skin. The FDA has approved 17 active ingredients for use in sunscreens.

Male Breast Reduction | Little Silver, NJIt’s not unusual for a woman with overly large natural breasts to seek to have reduction surgery. The reasons are often a mix of both physical and emotional, even social factors.

Conversely, while many men suffer from enlarged breasts, a condition clinically known as gynecomastia, few have the surgery with Dr. Hetzler to correct the problem. Instead, they simply avoid taking off their shirt or tolerate the “man boobs” jokes from friends.

It’s usually not simply due to weight gain

Gynecomastia, commonly known as abnormal male breast enlargement, is frequently associated with teenage boys. This is largely due to hormonal shifts around this period, as well as excess body weight gained throughout childhood. Recently, as the procedure has received more attention, older patients in their 50s have considered the procedure. The question is — is there a top age limit for male breast reduction? The short answer is — it depends.

There is no specific age limit in reducing the appearance of enlarged male breasts via surgery. Older men can still go through the procedure as long as they’re generally healthy and have no existing health issues that could potentially increase the risk of complications from anesthesia and the surgery itself.

Skin laxity can be a problem when Dr. Hetzler performs breast reduction on older men. After the excess glandular tissue is removed, the skin needs to tighten down to its new slimmer contours. Otherwise, the procedure is somewhat pointless. So, when you’re in for your consultation with Dr. Hetzler, he will assess how elastic or inelastic your skin is. The maturity that comes with the passing years also allows older patients to have a more realistic view of this surgery and what it can, and cannot, improve. So, know that age alone is not a deciding factor in whether or not male breast reduction surgery would be effective.

A quick look on the procedure

Male breast reduction surgery with Dr. Hetzler is not an overly difficult procedure. Tumescent anesthesia is used to lessen the pain and discomfort during and after surgery. First, liposuction via tiny incisions is performed to remove excess fat. Excess breast glands may be removed through a tiny incision made beneath the areola (the dark portion of nipples). The procedure can take from two to three hours with recovery lasting a week or two.

But Dr. Hetzler has three options for getting rid of your double chin: Kybella, CoolMini, or liposuction.

Kybella

Kybella is basically deoxycholic acid. In the body, the job of deoxycholic acid is to aid in the breakdown of dietary fat. So, when you stop at the nearby McDonald’s over in Shrewsbury, it’s deoxycholic acid you’re counting on to go after all the fat you’re eating.

The makers of Kybella figured out that deoxycholic acid doesn’t care where the fat it targets came from — it simply goes after it and breaks it down. They theorized that if deoxycholic acid were injected under the chin, where fat has accumulated and contributes to the sagging jowls, the deoxycholic acid would get to work. And so it does. And Kybella was born. When Dr. Hetzler injects Kybella into your jowls, the deoxycholic acid breaks down the fat it finds, dramatically slimming your profile.

CoolMini

Everyone has heard of CoolSculpting, the non-invasive procedure where fat cells are frozen, die, and are then flushed from the body. CoolSculpting came about after two Harvard scientists were studying the development of dimples in children who ate lots of popsicles. They theorized that the cold was somehow affecting the fat in the area creating dimples (where there is little fat over the underlying muscle).

CoolSculpting was the result. The scientists discovered that fat cells freeze at a higher temperature than the surrounding skin and muscle tissue. CoolSculpting delivers controlled cooling down into the treatment areas, and the fat cells freeze but the rest of the tissue remains undamaged. CoolSculpting permanently removes around 22% of the fat in the treated areas.

The problem was always dealing with small areas, such as the jowls or bra line fat. CoolSculpting’s original cooling panels were relatively large. CoolMini was the answer. Its applicator is much smaller; it’s perfect for areas such as the jowls.

Liposuction

The third option for removing fat under the chin is with targeted liposuction. Dr. Hetzler takes a very thin cannula and suctions out the unwanted fat of the jowls, slimming the profile. Liposuction continues to evolve into more and more finely targeted offerings, such as the jowls. There is no scarring and the results are permanent.

Chemical Peel Little Silver, NJIn New Jersey, that title sounds like a slogan for a Newark chemical company. But here in Little Silver, we mean it as chemical peels, which basically force your outer skin to exfoliate. Chemical peels remove dull, dead skin, revealing new, smoother, younger-looking skin beneath. We provide different degrees of chemical peels to suit the different goals of our patients.

What can you treat with a chemical peel?

Most of our chemical peels at Dr. Hetzler’s are done on the face, but they can also be used on the neck and the backs of the hands. Here are the skin issues they can address:

  • Lessen the appearance of wrinkles caused by sun damage and aging
  • Treat certain types of acne
  • Minimize fine lines, especially those under the eyes and on the corners of the mouth
  • Reduce the appearance of freckles, age spots, and melasma
  • Improve the overall appearance of the skin

Types of chemical peels

It can sound harsh but the goal of all chemical peels is to destroy certain parts of the skin to encourage the growth of new skin cells. With more than superficial peels, there is noticeable skin peeling that lasts from a day to two weeks. The duration depends on the penetration and type of the chemicals used in the peel.

  • Superficial peels— These are mild peels usually done with acids derived from fruits, called glycolic acids. Superficial peels may have some light peeling over the next day or two, but nothing extreme.
  • Medium peels— These peels usually involve trichloroacetic acid. They penetrate more deeply into the skin, causing a second-degree burn on the skin. There will be peeling and crusting with medium peels as the skin begins the process of rebuilding.
  • Deep peels— Phenol acid is used in deep peels, which are the deepest penetrating peel. They are only used on the face to avoid scarring elsewhere. These peels involve the most crusting of the skin and require the longest recovery time. Patients usually need to take at least one week off work, and will need to be very diligent about blocking their peeled areas from sun exposure. Deep peels also deliver the most skin rejuvenation.

Breast Augmentation Little Silver, NJIn New Jersey and seemingly around the rest of the country, breast augmentation continues to become more and more popular. For the past few years, breast augmentation has been the most popular cosmetic surgery in the U.S. In 2017, over 300,000 American women opted to augment their breasts. That was up three percent from 2016. The same is true with Dr. Hetzler’s patients, who have found this to be a rewarding procedure.

If you’ve been considering breast augmentation with Dr. Hetzler, here are five statistics you may be interested in.

#1 Extremely high satisfaction rates

Women who have had breast augmentation are routinely surveyed in the months and years afterwards. They report incredibly high rates of satisfaction. A recent survey from the American Society of Plastic Surgeons reports that 98 percent of women felt their augmentation either met or exceeded their expectations. Over 90 percent of the women also said they felt the procedure increased their self-esteem.

#2 Easy recovery

Breast augmentation surgery does not involve a difficult recovery. Most women are able to return to work after just a few days of recovery. The pain is not acute. Many of our patients equate the soreness to that from a rigorous upper body workout.

#3 Implants have proven to be safe

Before any implants were initially approved the FDA studied them extensively. Later, in the 80s, when there was some concern about silicone and safety, the FDA conducted another, even more rigorous, study and found no link between health issues and silicone breast implants. Plus, modern silicone gel is now very cohesive, meaning it usually adheres to itself and doesn’t leak out into the breast pocket if the implant ruptures.

#4 Duration

Implant durability is continuing to improve. Although most implants will need to be replaced at least once in a patient’s life, there is no specific timeline. Manufacturers guarantee their implants for at least 10 years, and many patients have implants that can last double that time. Plus, replacing implants is not difficult as Dr. Hetzler can simply enter through the same original incisions to remove the old implants and place the new ones.

#5 Variety of options

There are a variety of implants available from different manufacturers. These vary by the size, the shape, and the projection. This allows patients to choose an option that fits their goals, rather than adapting to a one-size-fits-all approach.
